Akutní peripartální hysterektomie – naše 6letá zkušenost

Overview

Cíl: Peripartální hysterektomie je život zachraňující zákrok prováděný během vaginálního porodu nebo porodu císařským řezem nebo po něm. Incidence abnormální invaze placenty se zvyšuje spolu s počtem porodů provedených císařským řezem. Cílem studie bylo zhodnotit četnost, rizikové faktory a výsledky peripartálních hysterektomií provedených v nemocnici poskytující terciární péči. Materiály a metody: Data pro výzkum byla získána na základě retrospektivní revize záznamů o pacientkách. Do studie byly zařazeny pacientky, které podstoupily peripartální hysterektomii z důvodu postpartálního krvácení na Gynekologicko-porodnické klinice Fakultní a výzkumné nemocnice Lékařské univerzity v Trabzonu v Turecku. Pacientky byly rozděleny do dvou skupin – na ty, které podstoupily akutní pertipartální hysterektomii (APH), a na ty, které nikoli. O pacientkách byly shromážděny demografické údaje, údaje o mateřské a fetální mortalitě, indikaci k APH, dalších zákrocích provedených v rámci APH a údaje o komplikacích během výkonu a po něm. Statistická analýza byla provedena pomocí Pearsonova chí-kvadrát testu. Výsledky: Celkem proběhlo 22 464 porodů, z toho 13 514 vaginálně a 8 950 císařským řezem. Peripartální hysterktomie byla provedena u 42 pacientek (z toho 16 mělo vaginální porod a 26 císařský řez). Nejčastější indikací k APH v obou skupinách bylo spektrum poruch placenta accreta (PAS) (42,9/ 3,2 %) a dále atonie dělohy (38,1/ 2,5%). Bylo zjištěno, že nejčastějším rizikovým faktorem pro APH byl předchozí císařský řez v anamnéze. Závěr: Příčinou abnormálních invazí placenty způsobujících závažné poporodní krvácení je vysoký počet prováděných císařských řezů. Abnormální invaze placenty je přitom nejčastější indikací k APH.
