Clinical Outcome, Inflammatory Responseand Tissue Trauma in Total LaparoscopicHysterectomy: Comparison toLaparoscopically-assisted Vaginal Hysterectomy
Klinické výsledky, zánětlivá reakce a tkáňovépoškození při totální laparoskopické hysterektomii –srovnání s laparoskopicky asistovanou vaginálníhysterektomií
Cíl studie:
Kvantifikace a porovnání klinických výsledků, zánětlivé reakce a tkáňového poškozenípři totální laparoskopické hysterektomii (TLH) a laparoskopicky asistované vaginální hysterektomii(LAVH).Typ studie: Klinická studie.Pracoviště: Gynekologicko-porodnické a biochemické oddělení, Nemocnice Kladno, Biochemickéoddělení, FN Praha-Motol.Metody: 58 pacientek vybraných k laparoskopické hysterektomii bylo selektováno k TLH neboLAVH na základě jejich volby a lékařského doporučení. Krevní vzorky k hodnocení markerůa tkáňového poškození (C-reaktivní protein, interleukin-6, kreatin, kináza, kortizol, cystatin C,serotonin a krevní obraz) byly odebrány před operací, první a třetí pooperační den. Jedna pacientkas operační komplikací (léze ureteru) byla vyloučena z laboratorní analýzy.Výsledky: Nebyly zjištěny žádné rozdíly v demografické charakteristice a klinických výsledcích(krevní ztráta, komplikace, délka hospitalizace) u 57 žen s laparoskopickou hysterektomií. Statistickyvýznamné rozdíly byly nalezeny pouze v délce operačního výkonu (70 min. v TLH skupině vs90 min. v LAVH skupině, pKlíčová slova:

Objective:
Our aim was quantify and compare the clinical outcome and surgical inflammatoryresponse and tissue trauma between the total laparoscopic hysterectomy (TLH) and laparoscopically-assisted vaginal hysterectomy (LAVH).Design: Clinical study.Setting: Departments of Gynaecology, Obstetrics and Biochemistry, Hospital Kladno, Departmentof Biochemistry, Faculty Hospital Motol-Prague.Methods: Fifty-eight patients scheduled for laparoscopic hysterectomy were selected accordingpatients or doctor preference to undergo either TLH or LAVH operative technique. Blood samplesfor assay of markers of tissue trauma (C-reactive protein, interleukin-6, creatine kinase, cortisol,cystatin C, serotonin and white blood cells count) were taken preopereatively, on the first andthird postoperative day. One patient with intraoperative complication (ureteral injury) was excludedfrom tissue analysis.Results: No differences were present in the demographic characteristics and clinical outcomes(blood loss, complications and hospital stay) in fifty seven laparoscopic hysterectomies.
