Preoperative lymphatic nodes detection using computed tomography in endometrial cancer patients

Předoperační detekce lymfatických uzlinpomocí computerové tomografi eu onemocnění karcinomem endometria

Cíl studie:
Stanovit přínos předoperační computerové tomografi e (CT) v predikci postižení lymfatickýchuzlin u pacientek s endometriálním karcinomem.Typ studie: Prospektivní multicentrická studie.Název a sídlo pracoviště: Gynekologicko-porodnická klinika, LF UP a FN, Olomouc, Gynekologicko-porodnická oddělení spolupracujících nemocnic.Metodika: Do studie bylo zařazeno celkem 164 žen s endometriálním karcinomem diagnostikovanýmv období 1998-2002. Bylo porovnáno defi nitivní histopatologické vyšetření s nálezy předoperačníhoCT pro zhodnocení postižení lymfatických uzlin.Výsledky: V souboru 164 žen s předoperačním CT vyšetřením a samplingem lymfatických uzlinbyla senzitivita, specifi cita, pozitivní a negativní předpovědní hodnota CT vyšetření při predikcipřítomnosti metastáz v lymfatických uzlinách 30, 95,5, 30 a 95,5 %.Závěr: Rutinní předoperační CT vyšetření má špatnou schopnost predikce postižení lymfatickýchuzlin u pacientek s endometriálním karcinomem. Jeho rutinní předoperační používánínelze doporučit.

Overview

Objective:
To determine the value of computed tomography (CT) in predicting preoperativelynodal disease in patients with endometrial cancer.Design: A prospective multicentric study.Setting: Department of Obstetrics and Gynecology of the Palacky Univerzity Medical School andUniverzity Hospital, Olomouc, Obstetrical and Gynecological Departments of collaborating hospital,Czech Republic. Methods: A total of 164 women with endometrial carcinoma that was diagnosed between 1998 and2002 were recruited for this study. Final pathological fi ndings were compared with those of thepreoperative CT scans to assess nodal disease. Results: Among 164 women who had preoperative scans and lymph node samplings, sensitivity, specifi city, positive and negative predicting values of CT scans at predicting lymph node metastasiswas 30, 95.5, 30 and 95.5% respectively. Conclusion: Routine preoperative CT scanning is a poor predictor of nodal disease in patientswith endometrial cancer. Its preoperative routine use should be discouraged.
