Diagnostics of Urethral Intrinsic Sphincter Deficiencyin Females
Diagnostika intrinzickej nedostatočnostiuretrálneho zvierača u žien
Ciele štúdie:
Stanovenie intrinzickej nedostatočnosti uretrálneho zvierača (ISD) podľa tlakovéhobodu úniku v populácii žien so stresovou inkontinenciou.Typ štúdie: Prospektívna klinická štúdia.Názov a sídlo pracoviska: Urologická klinika Jesseniovej lekárskej fakulty Univerzity Komenské-ho v Martine, Slovenská republika.Metodika: Do štúdie bolo zahrnutých 204 žien so symptómami dolných močových ciest. Vylúčenébolo neurogénne ochorenie a nestabilita močového mechúra. Tlakový bod úniku moču sa stanovilpočas Valsalvovho manévra (VLPP). ISD znamenala únik pri hodnote nižšej ako 65 cm H2O.Plienkový test (PWT) bol pozitívny pri úniku väčšom ako 2 g za hodinu.Výsledky: Stresová inkontinencia moču bola v 134 prípadoch a kontinencia (kontrolná skupina)v 70 prípadoch. Prvý stupeň inkontinencie bol v 83 prípadoch, druhý stupeň v 45 prípadoch a tretístupeň v 6 prípadoch. ISD bolo v 14 prípadoch (10,5 %), 8 prípadov s druhým stupňom a 6 prípa-dov s tretím stupňom inkontinencie. Najlepšia korelácia bola medzi VLPP a PWT, symptómamiinkontinencie a vekom žien. U všetkých ISD bola v minulosti chirurgická suspenzia uretry.Záver: ISD sa vyskytuje v druhom (6 %) aj v treťom (4,5 %) stupni v bežnej populácii žien so streso-vou močovou inkontinenciou.Klíčová slova:

Objective:
Evaluation of intrinsic sphincter deficiency (ISD) according to the urethral leak pointpressure in female population with stress urinary incontinence.Design: Prospective clinical study.Setting: Department of Urology, Jessenius School of Medicine Comenius University Martin, Slo-vak Republic.Methods: Study population compromise of 204 females with lower urinary tract symptoms. Exclu-sion criteria were neurogenic bladder and unstable bladder. Valsalva leak point pressures wereevaluated (VLPP). ISD was defined if VLPP below 65 cm H2O has been occured. Pad weighing test(PWT) was positive after leakage of 2 g per hour and more.Results: Stress urinary incontinence was in 134 and continence in 70 cases (control group). Thefirst stage of incontinence was in 83, the second stage in 45 and the third stage in 6 cases ofincontinence group. ISD has been occured in 14 cases (10,5 %), 8 cases with the second stage and 6cases with the third stage. The best correlation was between VLPP and PWT, symptoms of incon-tinence and age of patients. All ISD cases underwent antiincontinent surgery with urethral sus-pension, some years ago.Conclusion: ISD has been risen in the second stage (6 %) and the third stage (4,5 %), also, in thefemale population with stress urinary incontinence.Key words:
