Burch Colposuspension in the Treatment of Urinary StressIncontinence: Case Report


Kolposuspenze podle Burche v léčbě stresové inkontinence –kazuistika

Cíl studie:
Zhodnocení kolposuspenze podle Burche v léčbě stresové inkontinence u pacientky poradikální abdominální hysterektomii typ III, adjuvantní radioterapii a chemoterapii pro karci-nom děložního hrdla.Typ studie: Kazuistika.Název a sídlo pracoviště: Gynekologicko-porodnická klinika UK 2. LF a FN v Motole, Praha.Předmět studie: Vyhodnocení úspěšnosti kolposuspenze podle Burche při chirurgické léčbě stre-sové inkontinence u 54leté pacientky po radikální hysterektomii (Wertheim typ III) a adjuvantníradioterapii a chemoterapii pro karcinom děložního hrdla.Závěr: Stresová močová inkontinence po úspěšné radikální léčbě zhoubného nádoru děložníhohrdla významně snižuje kvalitu života žen postižených tímto onemocněním. Kvalitní peroperačnía pooperační management, pravidelné sledování, včasná léčba komplikací a správný psychologic-ký přístup se podílejí na minimalizaci potíží pacientek. Pokud přetrvává stresová inkontinencevíce než 1 rok od ukončení radikální léčby, potom vždy doporučujeme urogynekologické vyšetře-ní a ve správně indikovaných případech adekvátní chirurgickou léčbu. Kolposuspenze podle Bur-che byla s úspěchem použita v léčbě stresové močové inkontinence u pacientky po radikálníhysterektomii s těžkou postiradiační a postchemoterapeutickou fibrózou.

Objective:
The evaluation of Burch colposuspension in the treatment of stress urinary incontinence in patient after radical abdominal hysterectomy type III with adjuvant radiotherapy andchemotherapy for cervical cancer.Design: Case report.Setting: Obstetrics and Gynecology Department, Charles University 2ndMedical School and Faculty Hospital Motol, Prague.Subject: The evaluation of success rate and surgical technique of the Burch colposuspension inthe treatment of urinary stress incontinence in 54 years old patient after radical hysterectomyand adjuvant radiotherapy and chemotherapy for cervical cancer.Conclusion: Stress urinary incontinence after the successful radical therapy of the cervical malignant tumor significantly reduces the quality of life of the afflicted women. The quality of intraoperative and postoperative management, regular follow-up, early treatment of the complicationsand proper psychological access are important to minimalize the patient’s difficulties. When thestress urinary incontinence is present more than 1 year after the end of the radical therapy, wealways recommend urogynecological examination and proper surgical treatment in right indicated cases. Burch colposuspension is a very good surgical modality which was successfuly used instress incontinent patient after the radical hysterectomy with severe postirradiative and postchemotherapeutic fibrosis.
