Effect of the Number of Transferred Embryos on theMultiplicity of Pregnancy

Cíl studie:
Vliv počtu transferovaných embryí na četnost gravidity.Typ studie: Retrospektivní hodnocení výsledků centra.Název a sídlo pracoviště: Centrum asistované reprodukce při gynek.-porod. klinice FN a LF UP,Olomouc.Metodika: Hodnotili jsme 572 embryotransferů (ET) provedených v letech 1992 - 1998 na našempracovišti.Výsledky: Ze 173 získaných gravidit (G) bylo 56 % jednočetných, 35 % dvojčetných a 9 % trojčet-ných. Transfer jednoho embrya byl proveden u 53 pacientek (10 %), bylo získáno 15 % G/ETa všechna těhotenství skončila porodem. Po transferu dvou embryí (u 98 pacientek, 17 %) bylaúspěšnost G/ET 20 %, těhotenské ztráty (potrat - AB, mimoděložní těhotenství - GEU) činily 10 %.Z gravidit získaných po transferu dvou embryí bylo 55 % dvojčetných. Transfer tří embryí bylproveden u 357 pacientek (62 %, G/ET 36 %), v 13 % byla gravidita dvojčetná a ve 3 % trojčetná.V této skupině byly největší těhotenské ztráty u trojčetných gravidit (27 %). Čtyři embrya bylatransferována v 64 případech (11 %, G/ET 28%), dvojčetné gravidity tvořily 6 % a trojčetné 5 %.Veškeré těhotenské ztráty (27 %) byly u jednočetných gravidit.Závěr: Z rozboru vyplývá, že úspěšnost otěhotnění (G/ET) stoupá se zvyšujícím se počtem transfe-rovaných embryí. Při transferu tří embryí je dosažení vysokého poměru G/ET doprovázeno velký-mi těhotenskými ztrátami. Výhodnější se proto jeví ET dvou embryí, kdy jsou při dosažené nižšíúspěšnosti G/ET i nižší těhotenské ztráty a snížené riziko vícečetné gravidity. V souladu s celosvě-tovou tendencí snižovat počet transferovaných embryí a na základě zkušeností bude vhodné(s ohledem na individuální přístup) transferovat převážně dvě embrya.

Objective:
The aim of this retrospective study was to assess the influence of the number oftransferred embryos on multiple pregnancy.Design: Retrospective clinical study.Setting: Department of Obstetric and Gynecology FN UP, Olomouc.Methods: Over the last seven years 572 embryotransfers (ET) have been performed in our centre.Results: Of the 173 pregnancies (P) achieved there were 56% singletons, 35% twins and 9% triplets.One embryo was transferred in 10% of cases and all 8 pregnancies (15% P/ET) ended with a livebirth. Two embryos were transferred in 17% of cases. The pregnancy rate was 20% P/ET, 55% ofpregnancies were twins and 10% pregnancies were lost in abortion or ectopic pregnancy. Threeembryos were transferred in most cases (62%). The pregnancy rate was the highest in this group(36% P/ET) with 13% twins and 3% triplets. The overall pregnancy loss was 16%, the highest loss,27%, recorded in triplets. Four embryos were transferred in 11% of cases. The pregnancy ratedecreased to 28% P/ET, as well as the number of twins (6%), while the number of triplets increasedslightly (5%). Pregnancy loss occurred only in singleton pregnancies.Conclusion: We can conclude that in our setting the pregnancy rate increases with a highernumber of embryos up to three embryos transferred. Unfortunately, the higher pregnancy ratewith three embryos transferred is on the expense of a higher risk and losses due to multiplepregnancy. Therefore we recommed to transfer two embryos unless there are some specific rea-sons.
