Heterotopic Pregnancy, its Prevalence in Conjunctionwith Assisted Reproduction
Heterotopická gravidita a její výskytv souvislosti s asistovanou reprodukcí
Cíl studie:
Zhodnotit výskyt heterotopické gravidity v programu asistované reprodukce (AR).Typ studie: Prospektivní studie 618 klinických těhotenství po asistované reprodukci.Metodika: Soubor zahrnuje klinická těhotenství vzniklá metodami AR od 1. 1. 1997 do 30. 6.1998. Podmínkou zahrnutí do souboru je ultrazvukem detekovaný gestační váček a histologickypotvrzená ektopická gravidita.Název a sídlo pracoviště: Sanatorium Pronatal, Na dlouhé mezi 4/12, 147 00 Praha 4 - Hodkovičky.Výsledky: Z 618 klinických gravidit bylo 23 těhotenství ektopických (3,72 %), 7 z těchto graviditbylo heterotopických (30,5 %), což tvoří 1,14 % všech klinických těhotenství vzniklých metodamiAR.Závěr: Výskyt heterotopické gravidity se vlivem léčby sterility metodami asistované reprodukcestává relativně častou a život ohrožující komplikací gravidity. V našem souboru se vyskytla 1heterotopická gravidita na 88 klinických těhotenství. Prognóza pro intrauterinní těhotenstvív případě kombinované gravidity je obvykle dobrá. Okolo 76 % těchto plodů se rodí v termínuporodu.

Objective:
To assess the incidence of heterotopic pregnancy after infertility treatment using thetechnology of assisted reproduction.Design: A prospective study of 618 women who became clinically pregnant following assistedreproduction technology (ART) procedures.Setting: Sanatorium Pronatal, Na dlouhé mezi 4/12, 147 00 Praha 4 - Hodkovičky.Methods: Study group consists of clinical pregnancies conceived after ART procedures within theperiod from January 1, 1997 untill June 30, 1998. A condition to be included in survey group wasthat there was a gestation sac detected by ultrasound or histological confirmation of ectopicpregnancy.Results: Six-hundred-eighteen clinical pregnancies resulted and 23 of the pregnancies were ecto-pic gestations (3.7%). Seven out of the 23 (30.5%) ectopic pregnancies were heterotopic. Thusheterotopic pregnancy rate after ART was, 1.14% (1 in 88).Conclusion: The incidence of heterotopic pregnancy following assisted reproduction technique isrelatively frequent. This condition represents a live-threatening complications of pregnancy. Theprognosis for intrauterine gestation in case of heterotopic pregnancy is usually good. About 78%delivered living child at term.
