New Options in Pharmacological Treatmentof Stress Urinary Incontinence in Women


Nové možnosti v medikamentózní léčběstresové inkontinence moči u žen

Cíl studie:
Cílem tohoto článku je krátký aktuální přehled současné medikamentózní léčby pravéstresové inkontinence moči (GSI) u žen.Typ práce: Souhrnný přehled.Název a sídlo pracoviště: Gynekologicko-porodnická klinika VFN a 1. LF UK, Praha.Výsledky: Pravá stresová inkontinence u žen je mimovolný únik moči, který nastává při zvýšenínitrobřišního tlaku (např. při běhu, skoku, kašli atd.). Ačkoli cvičení svalů pánevního dna, změnanávyků, režimu a chirurgická léčba jsou v současné době základní léčbou GSI, domníváme se, žemedikamentózní léčba může pomoci zvýšit úspěšnost léčby u těchto žen. Nové možnosti použitíduloxetinu, inhibitoru zpětného vychytávání serotoninu (5-hydroxytryptamin[5-HT]) a noradrenalinu(NA) ve spinální míše vybízejí znovu posoudit medikamentózní léčbu GSI.

Objective:
The aim of this paper is to provide a brief update review of current pharmacologicalagents used to treat women with GSI (Genuine Stress Incontinence).Design: Review article.Setting: Gynecological and Obstetric Clinic, 1. LF UK and VFN, Apolinářská 18, Prague 2.Results: Genuine stress urinary incontinence is the involuntary loss of urine associated withincreasing of intraabdominal pressure (such as running, jumping, coughing, etc.). Although thepelvic fl oor muscle training, behavioral and surgical therapies are currently the mainstay of treatingGSI, we believe that pharmacological treatment can help to increase the cure rate. With theadvent of duloxetine, a serotonin (5-hydroxytryptamine [5-HT]) and norepinephrine (NE) reuptakeinhibitor acting within the sacral spinal cord, the role of pharmacological treatment of GSImust be reviewed ev. re-examined.
